Trinity CHANGE, Inc. offers a comprehensive supported living needs assessment with input from the individual, support people, family, friends, and a review of case history. Once all of the input is gathered, a recommendation for appropriate support is provided.
If Trinity CHANGE feels we have the capacity to provide
appropriate services, a support plan and personalized budget are created with
input by all members of the consumer's Circle of Support (friends/family/
supporters). The plan includes a proposed timeline, a transition calendar, and
specific information about the consumer being served.
Once the completed proposal is reviewed by the individual and his/her Circle of Support, the completed assessment is delivered to the Regional Center for approval.
